GVA 2025-26 Testing Waiver Forms

This form confirms your request to opt your student out of the assessments administered at Global Village Academy North for the 2025-2026 school year. (Please note: a separate form is required to opt out of CMAS assessments.)

This form confirms your request to excuse your child from the mandatory state assessments at Global Village Academy North in spring 2026. A new waiver must be submitted annually, and one form is required per student.

 

Please complete and submit the form by March 16, 2026, as it will no longer be available after this deadline.
